1 & 3 are correct.
2 & 4 are incorrect because you must use the infinitive for imperative sentences:
2) Huwag mong tingnan ako! = Don't look at me!
4) Huwag kang tumingin sa akin! = Don't look at me!

@pj1506
edit: I see I'm wrong. Apparently it is also acceptable to use contemplated aspect with huwag:
Huwag kang kumain. = Huwag kang kakain. = Don't eat.
Source: learningtagalog.com/ grammar/verbs/comman ds_requests_and_wish es/huwag.html

Personally, I will stick with the infinitive when I talk, because I like to think of huwag as a pseudo verb and pseudo verbs require the infinitive in this type of construction. Huwag appears to be an exception.

"Huwag mong titingnan ako ”

@pj1506

Tingnán mo ako ! (√)

Huwag mong titingnan ako ! (X)
Huwag mo akong titingnan! (√)

Tumingín ka sa akin ! (√)

Huwag kang titingín sa akin ! (√)
Huwag kang tumingin sa akin (√)

For 'Huwag kang sentences', both the infinitive (to + verb) and contemplated forms are correct.

Example:

Don't shout !
Huwag kang SISIGAW ! (contemplated)
Huwag kang SUMIGAW!
(infinitive)

@pj1506
Question: Are those sentences correct?

The first sentence "Tingnan mo ako" is definitely correct. This points out the pattern: Infinitive + Non-Focus Pronoun (mo) + Focus Pronoun (ako).

The second sentence "Huwag mong titingnan ako" is incorrect because it does not indicate the proper order of words in a sentence. Filipino language is just like other languages which follows a certain pattern of putting words in order to convey a clear message. With that being said, the correct sentence should be "Huwag mo akong titingnan". Here, it does follow the pattern "Huwag + Non-Focus Pronoun (without any linker like ng) + Focus Pronoun (with the help of a linking word like ng) + (Object) Focus Verb.

The two remaining sentences are both correct since they indicate the proper order of words and of course they both convey a clear message based on the standards of Filipino.
